Job Description Department: Nursing Title: Liver Transplant Staff Nurse
Accountable to:
Head Nurse, Area Supervisor, Nursing Supervisor, ACNO, CNO.
Reports to:
Charge nurse, AHN and Head Nurse.
Job Summary:
The liver transplant nurse provides
total nursing care to patients donate and receives liver. The staff
nurse working within the liver transplant is responsible for the delivery of a high
standard of patient focused care by administers all treatments,
medications, maintains complete accurate records of nursing assessments and nursing
care, assists the medical team pre-operative and post-operative
care, also has
expertise in recognizing and monitoring patient’s complications like rejection.
Responsibilities and duties:

Apply liver
clinical pathways.
· Conduct admission, discharge and transfer procedures and assist others with activities and patient care.
·
Provide information
to patient and family about the patient care.
·
Check the
proper signatures on donor documents.
· worked with
patients and donors to establish a daily routine of care after they received a transplant
or donated
· Maintain
the privacy and confidentiality of patient information and records at all times.
· Take medical history, summarize data and establish patient's nursing diagnosis/needs.
· Observe and
record signs, symptoms, and behaviors, including the physiological status of patient.
· Provide immediate
Post-operative care
· Observe and record vital signs. The vital sign is very important in checking the
general status of the patient. A change in vital signs may mean infection or early
signs of rejection
· Ensure safe and effective care for patients, provide care for
patient have different drains, tubes and lines
· Observe and record intake and
output to evaluate fluid and electrolyte balances.
· Close attention
to the output in the drainage attached to the patient post operatively.
· Monitor signs
of volume overload and administer diuretics as ordered.
· Monitor signs
and symptoms of bleeding, including checking stools for occult blood.
· Monitor respiratory
status and watch for compression due to ascitic pressure on the diaphragm or pulmonary
edema.
· Monitor signs
and symptoms of encephalopathy.
· Monitoring
for signs and symptoms of intracranial pressure.
· Protect the
patient's airway by taking measures to avoid aspiration.
· Executing physician orders for all assigned patients; Administer medications and treatments safely and appropriately.
· Helps others practice proper aseptic technique, isolation procedures, and infection control procedures.
· Monitor and
report lab results.
· Provide patient
with diet as per nutritional consultation.
· Administer
blood products as ordered.
· Be familiar
with using all technological equipments in the liver transplant unit as mechanical
ventilator, cardiac monitor, infusion pumps, and defibrillator.
· Document
nursing care in all file and assigned patient records for this issues.
· Evaluate care provided, including patient response.
· Demonstration to assess patient understanding and compliance with health care directives and teachings.
· Encourage
Range of motion exercises to maintain existing strength, which improve the patient's
rehabilitation process post-transplantation.
· Teach the
patient to stay away from sick people to avoid risk of acquiring infections
· Educate the
patient to avoid over-the-counter Drugs.
· Provide information
to patient and family about the patient home care.
· Know the following protocols:

Infection
control policies.
Educational Responsibilities:
Under the
direction of the Head Nurse and Clinical Department Nursing Educators:
· Participate in the clinical induction of all new nurses and support staff.
· Develop leadership skills to act as an effective role model.
· Takes responsibility for their own learning and development needs.
· Attend all required in-services training programs.
· Wherever possible, stay current with required training such as Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
Communication and Interpersonal skills:
· Follow the appropriate communication channels as per the department organizational chart.
· Should be able to communicate effectively both speaking
and in writing.
· Maintain professional appearance and dress code.
· Wear Identification badge when working.
· Adhere to policy for being absent or late.
· Must be think critically, crisis management and able to innovative.
· Always promote good unit rapport to maintain teamwork.
Qualifications:
Education:
·
B.S.N, Nursing Technical Institute or Nursing Diploma.
Experience:
·
1 year liver transplant experience is preferred.
·
Work in an
ICU or related area that will give nurse specific skills necessary to go into transplant
nursing.
Training:
· Must be registered with the Egyptian
Nurses Union and/or Professional Registrar of Origin.
·
Attend liver transplant education Program.
·
50 hours of mandatory annual training.
·
BLS certification
is mandatory and ACLS is preferred.

Heparinization
of Central Venous Line.

Assisting
in Extubation.

in Endo Tracheal Intubation.
10- Managing patients with immunocompromised patient.
11- Management of a patient received cyclosporine.
12- Management of a patient received steroid.
